NOTES:
1. Base Salary: A competitive “fair market value” first year base salary
of $327,267.
2. Minimum Productivity Bonus (1st and 2nd Year Only): The minimum
productivity bonus in the first year of employment is $75,000, paid as a
lump sum on the first day of employment; and in the second full year of
employment, Physician shall earn a Minimum Productivity Bonus of $75,000
payable in advance as a lump sum on the first anniversary of employment.
Maximum Productivity Bonus: The maximum productivity bonus in the first
year of employment is $140,257 which is the sum of the $75,000 minimum
productivity bonus and additional productivity bonus potential of up to
$65,257.
3. Comprehensive Benefits: A comprehensive benefit package including:
health and dental insurance, group life/accidental death insurance,
dependent life insurance, long-term disability insurance, pension
contributions, professional license fees, and continuing medical
education. [Hospital pays for employee health insurance premiums plus
50% of dependent coverage]
4. Liability Insurance: $4,957 in estimated annual premium expense for
$1 Million/$3 Million coverage, and no tail.
5. Moving Expenses: Hospital will reimburse moving expenses, up to a
maximum of $20,000.
6. J-1/H-1B Visa and Labor Certification Assistance: Hospital will
reimburse J-1/H-1B visa expenses and provide assistance with
establishing permanent labor certification up to a maximum of $20,000.
7. Education Loan Subsidy: Eligible education loan debt for qualifying
physicians will be subsidized based on the terms of the loans.
Qualifying loan principal paid by Physician will be reimbursed by
Hospital at the end of each 4-year contract period, not to exceed
$50,000 with a lifetime maximum benefit of $200,000. Accrued loan debt
subsidy is forfeited if employment is terminated prior to the conclusion
of each 4-year contract period

OTHER PERQUISITES:
Oregon Tax Credit: The State of Oregon offers an income tax credit of
$5,000 for physicians which is equivalent to earning roughly another
$9,000 in base salary not reflected in the numbers above.

Schedule: Daytime hours, 8:00 a.m. - 5 p.p. Monday - Thursday covering for the internists in town (all daytime and left-over overnight admissions, rounding, etc.) The physician will be on the internal medicine call panel that would initially be 1/6 but would increase to 1/7-9 with the arrival of our new internists (one already signed on).
